This seems to be a Cooperative society whose org structure is different from that of a FPCL.
Not sure the directors of a cooperative society need a DIN number.
we need to check with Auditor Sujatha.

1. DIN Number
2. PAN Number
3. Passport first page and last page
4. Address proof like driving license, voter id
5. valid Digital signature of your directors (USB pen drive )
charges : 500+ 90 (tax) per director
If your directors don't have a Digital signature in addition to the above it is 500 + 90 (tax)
Handling/Postage Rs. 150 per company
